Our Model of Care is the way we provide care and services. It’s how we keep You and your Care Team on the same page. It’s how we bring together all the care and services You need to look and feel your best.

Aunt Martha’s integrated model of care is designed with you – the patient – at the center. Our trauma-informed approach shifts the focus from “What’s wrong with you?” to “What happened to you?”

It all starts with your Primary Care Medical Home (PCMH), and the relationship you build with your primary care provider (PCP).

Your PCP and other members of your Care Team will ask questions to get to know you. The more they understand about you, they more they can do to customize a plan to help you reach your goals.

Our model cares for the whole person and addresses the social determinants

Based on your needs, your PCP and Care Team at your medical home can help you get many kinds of other services. Our model makes it easy to connect patients with:

  • Behavioral health services (therapy, counseling, psychiatry)
  • Specialty health care services
  • Substance use disorder (SUD) services
  • Community-based support services like basic needs (food, clothing), housing and shelter, and transportation
  • Education and employment services
  • Care for special populations
