Access and Reach
Meeting People Where They Are
- 97% of our patients and clients live below 200% of the federal poverty level.

Coordinated Support Beyond the Clinic
In FY2025, our Integrated Community-Based Services (ICBS) team managed 3,525 referrals connecting families to more than 300 external providers for essentials like food, housing, and mental health support. Through our am4you platform, internal referrals increased 22% year over year, which speeds handoffs between clinical, crisis, and youth programs.

Aunt Martha’s ranks among the Top 5 health centers in Illinois and has been a National Health Center Quality Leader for nine consecutive years (2017–2025). We consistently outperform state and national benchmarks on measures that matter to families and payers.

Our depression remission rate (35.1%) is more than double the Illinois average. Mothers receiving prenatal care at Aunt Martha’s are 38% less likely to deliver a low-birthweight baby compared to the state average.

Behavioral Health and Community Impact
- Mobile Crisis Response (MCR) answered 1,897 calls in FY2025, with 91.6% at the highest urgency level. 40% of youth avoided hospitalization through community-based stabilization.
- Comprehensive Community-Based Youth Services (CCBYS) supported 599 youth. At discharge, 87% of crisis cases were in a family or long-term arrangement. 89% of non-crisis cases increased protective factors.
- Reporting Centers provided structured, skills-based programs for 420 court-involved youth and achieved a 90% satisfaction rate.
- Positive Youth Development (PYD) engaged 5,000+ community members, distributed $40,000 in food aid, and achieved a 98% improvement in self-reported well-being among app users.
These programs strengthen mental health, family stability, and community resilience, three core determinants of health.
