Byron Street, Chicago<\/a>.<\/p>\n\n\n\n<div style=\"height:10px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Equipos de empleados de ambos organismos trabajar\u00e1n mano a mano con los pacientes y sus cuidadores para elaborar planes de atenci\u00f3n, coordinar las visitas y supervisar los avances hacia los objetivos personales del tratamiento.<\/p>\n<\/div><\/div>\n\n<div style=\"height:10px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p class=\"wp-block-paragraph\">\u00abEsta asociaci\u00f3n consiste en aportar valor a la comunidad. Eso es fundamental para todo lo que hacemos y en LSSI hemos encontrado un socio que comparte ese compromiso\u00bb, dijo Ra\u00fal Garza, Presidente y Director General de Aunt Martha&#8217;s.<\/p>\n\n<div style=\"height:10px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p class=\"wp-block-paragraph\">Desde el punto de vista del paciente, seg\u00fan Garza, los servicios de atenci\u00f3n primaria, enfermer\u00eda y psiquiatr\u00eda in situ de Aunt Marthase integrar\u00e1n perfectamente con los servicios de asesoramiento y terapia para pacientes externos que ya se prestan en el centro Portage Cragin de LSSI. El modelo integrado es la base del enfoque asistencial basado en el valor de Aunt MarthaMartha y se ha demostrado que da lugar a mayores niveles de cribado, mejor adherencia al tratamiento, menos visitas a urgencias y menos ingresos hospitalarios, factores todos ellos que contribuyen a mejorar los resultados y a reducir los costes.<\/p>\n\n<div style=\"height:10px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p class=\"wp-block-paragraph\">\u00abLos proveedores de servicios sanitarios y humanos tienen la responsabilidad de mejorar la salud y el bienestar de su comunidad\u00bb, dijo Mark A. El modelo de hogar de salud integrado de la agencia ofrece servicios coordinados y basados en el valor, proporcionando bienestar a toda la persona a trav\u00e9s de la integraci\u00f3n de la atenci\u00f3n primaria y de la salud conductual y de los v\u00ednculos con los servicios de apoyo adicionales que abordan los determinantes sociales de la salud.
