If You Have an Upcoming Appointment,

Do This Quick Self-Assessment. 

Have you traveled to China, Iran, Italy, South Korea, Japan or Hong Kong in the last 14 days?

OR

Have you been in close contact with an ill person known to have or is suspected to have Coronavirus?

Are you experiencing fever, cough, or shortness of breath?

What Should You Do?

If you answered NO to ANY of these questions, we look forward to seeing you at your upcoming appointment. 

Reduce Your Chance of Getting Sick: 

Wash Your Hands Regularly, Especially After Coughing or Sneezing

Wash your hands with soap and water for at least 20 seconds, or use an alcohol-based hand sanitizer with 60 to 95% alcohol. When using hand sanatizer, be sure to cover all surfaces of your hand and rub your hands together until they are dry. Washing hands with soap and water is best, especially when hands are visibly dirty. 

Cover Your Mouth and Nose When Sneezing or Coughing

Sneeze or cough into a tissue and throw the used tissue into a lined trash can. Wash your hands immediately with soap and water for at least 20 seconds or use an alcohol-based hand sanitizer with 60 to 90% alcohol. When using hand sanitizer, cover all surfaces of your hands and rub them together until they are dry. Washing hands with soap and water is best, especially when hands are visibly dirty. 

Avoid Touching Your Face

Avoid touching your face, especially your eyes, mouth, and nose.
